Start with what you want to do again

Walking the neighborhood, returning to work, climbing stairs or playing a sport are more useful starting points than a vague goal to feel better. Write down which activities are difficult, what has changed and what you hope to regain. A physical therapist can use that conversation alongside an evaluation to discuss a plan.

Ask the office what clothing to wear, what records to bring and whether there are forms you can complete beforehand. Explain accessibility needs and any help you require getting into the office.

Understand the plan before filling your calendar

• What goals will we work toward, and how will progress be measured?
• How often are visits proposed, and when will the plan be reassessed?
• Who will provide care during appointments?
• What should I practice at home, and can you check that I understand the instructions?
• Who should I contact if an activity causes a problem or the plan is not working?

Confirm access and coverage

Before the first visit, check referral requirements, network status, authorization rules and visit limits with the practice and insurer. Ask how the office handles changes to the plan and what you may owe. The ability to book an appointment and the insurer’s payment rules are separate questions.

For people recovering after a hospital stay or considering care at home, ask the discharge or referring team which setting is appropriate and who coordinates the referral. Do not assume every rehabilitation practice offers the same services.
